What to Bring
- Insurance Information: Your dental insurance card (if applicable) so we can verify your benefits
- Dental Records: Any recent X-rays or dental records from your general dentist (if available)
- Photo ID: A valid driver’s license or identification card
- Patient Forms: Complete our online forms before your visit to save time (or arrive 10 minutes early to complete them)
- Questions: A list of any questions or concerns you’d like to discuss
For Parents & Guardians
If you’re bringing a child or teen for their first visit, we encourage you to accompany them throughout the appointment. We’ll make sure to explain everything in age-appropriate terms and answer all questions from both you and your child.
We want your child to feel comfortable and excited about their orthodontic journey, and we’ll take the time to ensure everyone understands the recommended treatment plan.
